Quality Improvement Projects

The Northeast Health Care Quality Foundation (NHCQF) is contracted by the Centers for Medicare and Medicaid Services (CMS) to work with practices with Electronic Health Records (EHRs) on several topic areas in the physician office setting. Progress is measured by the following quality of care measures by reporting and improving them:

 

Adult Immunizations

  • Influenza immunization
  • Pneumococcal (PPV) immunization

 

Cancer Screening

  • Biennial screening mammography
  • Colorectal cancer screening

 

As NHCQF moves into the 10th SOW we will continue to work in these areas on a state-wide basis. Additionally we will be performing two additional tasks.

 

CMS Physician Quality Reporting System (PQRS)

NHCQF is tasked with assisting practices to submit data via their Electronic Health Record (EHR) to CMS for the pay for reporting initiative PQRS, the Physician Quality Reporting System. NHCQF will assist practices that qualify to participate in this initiative. Only practices with specified EHR products and a specified version of that product are eligible for this initiative. Cardiac Population Health

NHCQF is developing a learning and action network to address cardiac population health. We will recruit practices interested in working on areas of cardiac health including Blood Pressure management, lipid management, aspirin therapy and smoking cessation. This network will be delivered both virtually and in person.

Adverse Drug Events

NHCQF is recruiting practices to work in the area of adverse drug events. Expanding on the work performed by Health Resources and Services Administration (HRSA) and their Patient Safety and Clinical Pharmacy Services Collaborative (PSPC). NHCQF will be working with Practices and Communities to address various areas in the realm of adverse drug events.
