|
The outpatient diabetes
management project is part of a national quality improvement initiative.
Diabetes Mellitus is a chronic disease that has a major impact on people 60
years of age and older. Non-insulin
dependent diabetes, most common in this population, can result in
debilitating complications. These
complications include blindness, kidney failure, neuropathy and vascular
disease. Recent studies indicate that
these complications can be reduced through early identification and
treatment. The goals of this project are: • Annual HbA1c Testing • Biennial testing of lipid profile • Biennial retinal exam by an eye
professional Additional goals include: • Micro–albuminuria screening • Annual neuro-sensory foot exam • Diabetes self-management education Our project plan is as follows: 1. Support
each practice with an on-site visit to discuss project details, specific
practice needs, and current office workflow. 2. Collect
data, through medical record review, to assess current trends within your
practice regarding the care of diabetic patients. 3. Present
baseline data to your group to identify and prioritize opportunities for
improvement. 4. Support
practices in integrating new tools and processes into current office systems
by providing: • On-site
visit and practice-specific support • Patient
management tools (flow sheets, reminder mechanisms, tracking systems, etc.)
and patient education materials. • Assistance
with office processes change. 5. Interim support and data feedback. 6. Final data measurement and presentation
to assess success. Currently, one in five
Medicare beneficiaries are affected by diabetes. Our ultimate goal is to improve their longevity and quality of
