Home Health Agencies

 

Quality Improvement Projects

 

CMS has directed its nationwide network of Quality Improvement Organizations (QIOs) to work collaboratively with home health agencies in each state to implement the Outcome-Based Quality Improvement (OBQI) system. The goal of the OBQI system is to use OASIS-derived outcome information to improve patient care processes and specific clinical outcomes.

 

Northeast Health Care Quality Foundation (NHCQF), the Medicare-contracted QIO for Maine, New Hampshire, and Vermont, has provided formal and informal training sessions on OBQI and will continue to provide ongoing technical support to home health agencies in the tri-state area in this effort to improve care to home care clients. 

 

NHCQF offers Home Health OBQI Process guidance at no cost to agencies.  We have trained 100% of the agencies in our tri-state area and have worked with each agency on their plans to improve care in their selected target outcomes.
